There are some free market idea that I like that would improve the NHS taken largely from Germany (rather than the mess that is the US).

First I would be tempted to try and see the situation when we spend a comparable amount, and if this improves the statistics. Money is not everything but increased spending would help. Obviously smart targeted spending would be best, if that does not work evidence based changes rather than gut feeling changes that this government has been guilty of. Look around choose the best out there and copy it.

Never thought I would say this but I have slowly been coming to terms with ID cards, it goes against my principles but it would certainly help in removing NHS tourism, most of EU are reimbursed for treatment under EHIC we are unable to, and the black/grey economy.

It's the emphasis on proactivity that comes with an insurance-based system. This scenario is not uncommon:

Canada/Germany - six monthly health checks. Tests pick up some - it's cancerous. Simple treatment. Back at work and healthy within weeks.

UK - collapse on the floor in A&E with a mystery pain/illness. Expensive acute treatment. Dead within a month because cancer is so advanced.
 
I know you need ot take Cuba numbers with a bit of a pinch of salt, but it is not insurance based, is very efficient and concetrates on preventative

US - if you are Rich great. If you can only afford the basic level same as the UK or worse as you are probably bankrupt with the excess. (*and it costs more than NHS).

Pump in the extra funding compared to Canada / Germany focus it on preventative care, after 5 years if it doesn't work then have evidence to change to a desired system. At least politically you can sell that argument, as right now the "against" can point to the differences in spending.
 
Self employment is good, however I think something needs to be resolved with regards to "self employment" / "contractors" - allows big companies to move employment costs (pensions / sick pay etc).

Not a discussion on whether companies should pay NI / Holiday pay / Sick pay (that is a larger discussion), as a society we have decided that this is the best way to finance these, my view is this is a loophole and we should try to close it.

didn't want to use Cuba due to the connotations and I am not making a judgment on the politics of the country but to chose a non insurance based example was always going to be left wing and it is the one that came to mind. Just using it as an example of non insurance based that is efficient and preventative.

That's if your lucky, there was a report on the local news the other day about how many folks have to wait in the ambulance at the hospital before they can be admitted into A/E. In 2014 the numbers were just 3,000 in the year, this year ( only two years later) the numbers had jumped to over 13,700.

And they say the NHS is in safe hands.
 
Even ring fenced NHS (?!?) matters not if they are removing funding of a lot of social care. I.e. remove Social taxi service means that NHS spend on sending ambulances etc. So effectively it is a cut, having to do more with less.

And as above we spend significantly less than most comparable as it is.

I agree in general, but don't think funding is anything like the be all and end all, when you see how inefficiently everything is run.

More prescriptions of exercise over drugs would cut spending significantly for starters. Take 2 laps of the park 4 times a week and come and see me again in a month if things aren't improving. Better triaging and more direct referrals would save a lot of wasted resources too.

:)I do not do fun stuff mate, one of the things that turns sane ( usually) people into moaners is armchairs supporters who think that because they can win the Universe cup on a abacus game then the game in the real world is as easy. :)

You're probably right that there is a lot of wastage, but if we are wasting a significant amount and significantly underspending compared to France and Germany would that imply we are not spending near enough in other areas.

I know nothing about actual funding, underspend / overspend but starting point for me would be comparisons to our peers. I chose France Germany as I would have them closest (size /in Europe etc)
